If you received an email asking for additional information in order to be eligible to receive payment, make sure you log in to the Remedy Claim portal to make any requested updates to your claim (by no later than midnight (ET), July 25, 2024). Note the subject of the email is titled: “Additional Action Required to Validate Your Claim”.

If you would like to submit an appeal of this decision, you must do so no later than August 1, 2024. Please enter below your full name, mailing address, a statement of reasons indicating your grounds for contesting the rejection, and upload any documentation you would like to submit in support of your appeal (for example, any proof of purchase for the Chicco “KidFit” booster seat(s) you claimed you purchased).
